The freeze

Even a decent eval still needs a one-pager, an ask, an if-no, and a same-day follow-up. Those are separate documents. This packet has them.
How a bullet is built
Every serious line in the self-eval should be reconstructible into these five parts. Compress them for the form. Keep the labels in your private draft until the last cut. No invented percentages.
What was true at the start: a gap, a goal, a volume, a constraint, a date. Not your childhood in the role.
What you did. Verbs. The boundary of you vs. the team. Artifacts by name if they help a reader find the work.
Where a skeptic looks. Link, doc title + date, ticket, dashboard, email from a role on a date. No pointer, no claim.
What is different. A real metric or a concrete qualitative change. In-flight work is labeled in-flight — not parked as a win.
What you would repeat or change. Missed goals live here, in facts. Next-cycle goals are written last.

Days 12–14. Two agendas. Dry-run out loud. Have the conversation. Send the follow-up the same day. Update the log — that row starts the next cycle.
